When you act as a juror, you must provide your social security number to the District Court using this Form AO 229S.
- The Federal Judiciary is required to file an information return with the IRS for any juror who is paid $600 or more in juror attendance fees during a calendar year.
- Because of that requirement, each juror is asked to provide his or her social security number so that it is on file with the Court in the event an information return needs to be filed.

Request for Excuse | USA
US District Court Form AO 224 is issued by the Clerk of the Court to a juror who wishes to be excused from jury duty.
- The Office of the Clerk is requesting that the juror submit the request to the Court along with the reasons they wish to be excused.
- If the reasons are because of medical conditions, a letter from the juror's doctor must be submitted.
