If your landlord has failed to perform its obligations, terminate your rental agreement with this Alberta Notice of Termination of Monthly Tenancy for Substantial Breach.

• You must serve the Notice on your landlord in accordance with the Residential Tenancies Act.
• The landlord is notified that the monthly tenancy will terminate in 14 days due to the landlord's substantial breach of the tenancy agreement, and an order has been issued under Section 62 of the Public Health Act.
